Why Does Your Staff Keep Making the Same Mistake?

When the same mistake keeps happening, shouting louder usually does not fix it. First find out whether the problem is skill, attitude, memory or a bad process.

Ask them to show you the task

Do not only ask if they understand. Let them do it while you watch.

You may discover

• A step is confusing
• The SOP is outdated
• They were trained wrongly
• They are rushing

Create a simple checklist

For repeated work, a short checklist can prevent many avoidable mistakes.

Good checklist

• Short
• In order
• Easy to tick
• Updated when the process changes

Know when it is no longer a training issue

If the person understands the process but continues ignoring it, the problem may be attitude or accountability.

Then you need

• Clear feedback
• Documented expectations
• A reasonable improvement period
• Proper HR action if needed

My view

Do not redesign the whole company because one person refuses to follow a clear process. But do not blame a person for a process that nobody can follow either.
